HOST HOMES
​
This is an arrangement in which an individual with a developmental disability chooses to live with an individual, couple, or a family in the community to share their life experiences together. A host home could be a single person, a college student, empty nesters, single parent or two-parent family with children.
INDIVIDUALIZED SUPPORTED LIVING (ISL)
​
This service includes individualized supports, delivered in a personalized manner, to individuals who live in their own home. ISL Services include assistance with activities of daily living (ADL) such as bathing, dressing, housekeeping, shopping, transportation, medication administration and meal preparation. This service also includes protective oversight and general supervision. This service is designed to assist individuals with residing successfully in their own home and community.
RELIEF
​
Support provided on a short term basis to individuals unable to care for themselves in the absence of their caregiver. Relief is also designed to allow the caregiver a period of rest.
PERSONAL ASSISTANCE
​
This service includes assistance with any activity of daily living (ADL) such as bathing, dressing, housekeeping, shopping, transportation, medication administration and meal preparation. This service also includes protective oversight and general supervision. The Personal Assistant may directly perform some activities and support the individual in learning how to perform others.
REGISTERED NURSE CONSULTANT

This service is provided by a Licensed Registered Nurse through face to face evaluation and consultation to promote the optimal health of individuals. Our agency RN monitors the health and safety of each indivudal we serve.
SUPPORT BROKER
​
This service is designed to assist those individuals who are self-directing their own services. The Support Broker provides assistance with employer related duties such as skills training related to interviewing, hiring, managing and training employees.
